Job Summary and Responsibilities
As a Certified Nurse Assistant (CNA), you will be a cornerstone of patient care, directly contributing to comfort, safety, and recovery by providing essential support under RN direction.
Every day, you will deliver compassionate basic care, assist with patient transport, and actively contribute to the patient's plan of care as a vital member of our interdisciplinary team, upholding the highest standards of conduct and patient well-being.
To be successful in this role, you will possess a dedicated spirit of service, strong communication skills, a meticulous approach to patient support, and an unwavering commitment to our mission of dignified, high-quality healthcare.
- PATIENT RIGHTS:
Demonstrates an understanding and respect of patient rights and responsibilities. Provides care in a manner that is cognizant of, and respectful towards the religious, cultural, linguistic and ethnic belief systems of the patient and their identified family structure. Obtains translation services for oral communication in the patient's primary language as needed; documents services utilized in the patient records. STANDARDS 1.Competency and appropriate care regarding religious, cultural, linguistic and ethnic belief systems of the patient/family is assessed by one or more of the following: direct observation, listening and auditing of documentation for the use of interpreters services. 2.There are no serious occurrences of valid patient or family complaints regarding privacy or confidentiality. 3.There are no breaches of the facilities policy and procedures regarding patient rights - PATIENT ASSESSMENT:
Data Collection Data collection is done in accordance with and within timeframes specified in MMC polices. Recognizes normal and expected findings appropriate to the developmental age (infant, toddler, preschooler, school age, adolescent, early adult, middle adult and geriatric) of the patient. Recognizes significant abnormalities and intervenes in a developmentally age appropriate (infant, toddler, preschooler, school age, adolescent, early adult, middle adult and geriatric) manner and reports them to the RN. Collects information to contribute to database to identify patient's high risk for falls and/or skin breakdown. Utilizes appropriate developmental age (infant, toddler, preschooler, school age, adolescent, early adult, middle adult and geriatric) tools and criteria in the assessment of pain. Performs care within the scope of the CNA as directed by the nurse. Provides for patient safety and comfort, practices infection and environmental control. Prioritizes tasks based on patient needs. Participates in patient safety goals & quality initiatives. Understands and can apply criteria to detect possible victims of abuse. STANDARDS 1.Developmental age competency is assessed by one or more of the following methods: direct observation, listening and auditing of documentation. 2.Competency and appropriate care regarding religious, cultural, linguistic and ethnic belief systems of the patient is assessed by one or more of the following: direct observation, listening and auditing of documentation for the use of interpreters services. 3.Data is documented properly 95% of the time based on random sample review of charts. 4.There are no occurrences of valid serious complaints or incident reports of failure to notify the nurse of changes in patient's condition in a timely manner or failure to communicate and utilize technical skills.Planning - as directed/delegated by the RN Performs care at required intervals in an age appropriate (infant, toddler, preschooler, school age, adolescent, early adult, middle adult and geriatric) manner. Completes and documents care provided in a timely manner. Provides and practices infection control standards for self and patient Organizes and prioritizes tasks as assigned to expedite patient care. Applies knowledge of hospital and unit-based policies and procedures to provide safe and competent care. As directed by the RN renders basic developmentally age appropriate (infant, toddler, preschooler, school age, adolescent, early adult, middle adult and geriatric) care to the patient. Provides ongoing communication to the nurse regarding obstacles to delivering timely care or the completion of tasks. STANDARDS 1.There are no occurrences of valid serious patient complaints or incident reports of failure to assist in the care of the patient. 2.Ensures accurate and complete documentation of patient records 95% of the time based on the random chart review. 3.Developmental age competency is assessed by one or more of the following methods: direct observation, listening and auditing of documentation. 4.There are no occurrences of valid serious complaints of failure to communicate to the nurse any observed changes in the patients condition or the inability to perform an assigned task timely.
Implementation Performs Patient Care Rounds at prescribed time intervals and includes all appropriate steps. Assists in the implementation of the plan of care as assigned with members of the healthcare team. Accurately plan and implement services that respond to the patient's cultural & linguistic needs. Address patients cultural and linguistic needs as appropriate and directed by the nurse. Assists in implementation of pathways and protocols per policy and procedures as directed by the nurse (i.e. Daily Weights and accurate I&O's). The visitor security policy and protocol will be maintained beginning with admission. Applies knowledge of hospital and unit-based policies and procedures to provide safe and competent care. STANDARDS 1.There are no occurrences of valid serious complaints or incident reports of failure to assist in the implement the plan of care with members of the health care team. 2.There are no occurrences of failure to follow hospital and unit based policies and procedures. 3.Developmental age competency is assessed by one or more of the following methods: direct observation, listening and auditing of documentation. 4.Cultural & Linguistic competency is assessed by one or more of the following: direct observation, listening and auditing of documentation for the use of interpreters services.
Evaluation Evaluates and prioritizes delivery of assigned tasks based on patient care needs. Effectively assists in managing patient receiving bedside procedures or post-op; reports any issues to RN/LVN. Documents accurately and completely all patient records such as vital signs, I&O sheets and flow sheets. Answers call lights in a timely fashion. Evaluates room for clutter; removes and cleans out old and unnecessary items. STANDARDS 1.There are no occurrences of valid serious complaints or incident reports of failure to appropriately assist or report any changes to the RN/LVN. 2.Ensures accurate and complete documentation of patient records 95% of the time based on the random chart review. 3.There are no occurrences in failing to assist license staff in caring for a patient receiving bedside procedures or post-op; or report any issues to RN/LVN. 4.Developmental age competency is assessed by one or more of the following methods: direct observation, listening and auditing of documentation.
- PATIENT AND FAMILY EDUCATION:
Reports to the RN/LVN any perceived significant learning needs, barriers to learning, learning and language preferences. Educates the patient and family regarding rounding, plan for the day or ADL's. Educates the patient and family to room, visitation policy and nursing routine upon admission. STANDARDS 1.Educates patient and their family regarding visitor policy. 2.Developmental age competency is assessed by one or more of the following methods: direct observation, listening and auditing of documentation. 3.Cultural & Linguistic competency is assessed by one or more of the following: direct observation, listening and auditing of documentation for the use of interpreters services. - CONTINUUM OF CARE:
Transmits patient specific data and/or information among and between members of the healthcare team in an accurate & timely manner to promote continuity of care within and between levels of care. Understands and utilizes the chain of command effectively as needed. Documentation is accurate and completed in a timely fashion. Participates in the Dignity Health Care Management Initiatives and completes the required tasks and documentation (i.e. Nursing Bundle, patient care rounds). STANDARDS 1.Ensures accurate documentation of information transmitted among and between health care team members 95% of the time based on random chart review. 2.There are no occurrences of valid serious complaints or incident reports of failure to contribute or to collaborate with the other health care team members.Teamwork Behavior Maintains team structure by respecting the established leader and cultivating the team climate in a positive way. Resolves conflict by addressing the team or team members in a professional manner any concerns. Participates in the planning and problem-solving situation. - HIPAA:
Basic expectations of the Employee based upon Corporate HIPAA compliance: Patient Privacy: Demonstrates work place practices that maintain patient privacy and confidentiality. Consistently follow regulatory, JCAHO, Dignity Health, and hospital policy requirements relating to the disclosure of and/or access to Protected Health Information (PHI). Review Network Usage Policy and signed acknowledgment form annually. Patient Information Security: Demonstrates work place practices that maintain the security of protected health information including maintaining PHI in a secure work environment, reporting suspected breaches of PHI security to Management or Privacy Offices, and following all policies and regulatory requirement for the safeguarding of PHI.
Required:
- completion of a nursing fundamentals class and are currently enrolled in nursing school, upon hire or
- a graduate from a nursing program, upon hire
- Must be able to take call, float to other areas, and work on weekends as required, upon hire
