Responsibilities

As a Hospice Home Health Aide, you are more than a caregiver – you are a cornerstone of comfort, providing essential support that allows our patients to live their final chapters with dignity in their own homes.

As a Hospice Home Health Aide, you will provide essential, hands-on care, ensuring dignity and well-being for our patients. Under the guidance of a Registered Nurse, your daily impact will include:

  • Personal Care & Comfort: Assisting with personal hygiene, mobility (ambulation and transfers), and promoting overall patient well-being.
  • Patient Monitoring: Diligently reporting any changes in condition to the nursing team.
  • Supportive Environment: Performing light housekeeping, preparing meals, and ensuring a safe, healthful, and comforting home environment.

Qualifications
  • High School Diploma or equivalent required.
  • Certified Nurse Assistant (CNA) in the state of Minnesota (MN) required. 
  • Basic Life Support (BLS) required. 
  • Must have a valid Driver’s License.
  • Must have current vehicle insurance.

Fully accredited by The Joint Commission, we offer a full range of services including 24/7/365 Emergency Department and Level IV Trauma Center, hospitalist program, testing, surgery, orthopedics, rehabilitation, and more so that you can experience better health.
