The posted compensation range of $53.322 - $79.32 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. At CHI Health at Home Infusion Pharmacy, we strive to embody our mission of delivering hope and healing to those we serve. As a Clinical Pharmacist, you will provide safe and appropriate pharmacy services to home infusion patients, including assessing patient suitability, reviewing medication orders and labs, monitoring progress, and ensuring quality assurance in sterile compounding.

  • Monitor and evaluate patient-specific pharmacy care plans for home infusion pharmacotherapy. Collect, organize, and analyze patient data to prevent and resolve medication-related issues. Document outcomes in CPR+ and adjust care plans based on evaluation.
  • Receive medication orders from physicians, evaluate the appropriateness of orders including dosage, drug choice, potential drug interactions, drug-food interactions, route of administration, and stability.
  • Ensure medication-specific laboratory tests are ordered and results received. Responsible for retrieving, reviewing, and consulting with physicians and nurses on ordered labs, utilizing critical lab and critical value reports.
  • Prepares and dispenses medications, selecting cost effective and patient safe and appropriate methods of infusion, following Infusion’s policies and procedures.
  • Manage the use, maintenance, and troubleshooting of all pharmacy equipment, with the ability to accurately program, maintain, and troubleshoot all IV medication-related equipment.
  • Ensure the continuity of pharmaceutical care from pharmacy through the delivery process.
  • Communicate pertinent information to patients, caregivers, and the nursing department.
  • Provides medication-related education to Infusion staff, nurses, physicians, patients and caregivers.
  • Assists the physician, nurse, patient, caregiver and Infusion staff with continued technical and clinical support. Provides accurate, concise and timely responses to clinical questions from health care providers, patients and caregivers.
  • Accurately documents all communication with physicians, nurses, patient, caregivers and other medical personnel in the electronic medical record and patient chart.
  • Assists in the training and supervision of pharmacy technicians, pharmacy externs and delivery personnel.
  • Participate in Performance Improvement (PI) Program Coordination, including assisting with the development and implementation of the PI plan, identifying goals and related patient outcomes, participating in activities and outcomes, and developing corrective plans.
  • Participate in the on-call rotation to ensure continuous pharmaceutical support and assistance outside regular working hours.
  • Participate in pharmacy team meetings.

Job Requirements

  • Registered pharmacist with state licensure required.
  • Minimum one year experience in a hospital pharmacy.
  • Experience with IV admixture procedures required.
  • Excellent oral and written communication skills.
  • Flexible and able to work in a multidisciplinary team.
  • Excellent time management skills and ability to work independently.
  • Ability to function well in a team.
