The posted compensation range of $59.36 - $67.16 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.

Job Summary and Responsibilities

The incumbent, as a member of the multi-disciplinary team, provides the following services to patients and/or families involved with MSJMCs Cardiac program: psychosocial assessment, counseling, exploration of effective coping skills, education, and coordination of the patient's progress through the continuum of care.

As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges. Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being. To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment.


Job Requirements
  • Masters in Social Work from a university or college accredited by the Council of Social Work Education.
  • Acute care hospital experience or a combination of hospital and other clinical experience which provides for the knowledge, skills and abilities listed.
  • Licensed Clinical Social Worker: CA
  • Basic Life Support from the American Heart Association.

Knowledge of: 

  • The principles and practices of clinical work in the healthcare setting.
  • Individual and group consultation process.
  • Principles of grief and loss counseling.
  • Crisis intervention techniques.
  • Behavior modification techniques.
  • Patient's rights, ethical considerations, cultural differences in dealing with the cardiac patient.
  • Community health, welfare and social agencies, chemical dependency and mental disorders, basic cardiac diseases and procedures.
  • Ability to: Counsel patients and families for emotional, social and physical issues by assisting them in coping with feelings/fears re: their diagnosis.
  • Demonstrate knowledge and skills relevant to the principles of growth
  • and development over the life span of the patient and interpret
  • knowledge needed to identify each patient's requirements relative to
  • his/her age specific needs.
  • Perform psychosocial assessment and formulate an appropriate plan.
  • Develop and maintain an empathetic environment with patients and families. 
  • Communicate effectively with the patient, family, interdisciplinary team members and others involved in the care of the patient, in a professional manner.

Dignity Health Mercy San Juan Medical Center is a 384-bed not-for-profit Level 2 Trauma Center located in Carmichael California. We have served north Sacramento County as well as south Placer County for over 50 years. Our facility is one of the area's largest medical centers and also one of the most comprehensive. Our staff and volunteers are dedicated to community well-being; providing excellent patient care to all.
