The posted compensation range of $27.97 - $41.60 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.

Job Summary and Responsibilities
  • Uses social work expertise to provide care management across the continuum for identified highest risk (complex) patients.
  • Proactively acts as a patient’s advocate, responding to and working to resolve concerns
  • Provides health promotion activities and education for patients
  • Optimizes patient and family independence through providing education and links to community resources
  • Works with families and patients on needs that may or will affect the patient’s health, including transportation issues, financial concerns, end of life planning, etc.
  • Identifies potential barriers related to patient’s home setting and self-care/management needs and works with patient (and caregiver) to resolve issues
  • Maintains an active case load of “at risk” patients using identification and stratification tools, direct referrals from the patient’s healthcare team (Physician or Provider, RN Population Health Coach, etc.) and other knowledge

We are offering a $20,000 sign on bonus to hires who meet the eligibility requirements. We also offer referral incentives as well as relocation assistance. 


Job Requirements

Master’s Degree in related field.
A current unencumbered Master Social Worker license to practice in the State of Arkansas is required.  

Provisional licensure NOT accepted.
BLS within 30 days.
