The posted compensation range of $43.10 - $64.11 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Job Summary and Responsibilities

As our Care Coordination Supervisor, you will be a pivotal leader, ensuring strategic optimization and seamless execution of critical care coordination functions.

Every day, you'll oversee and coordinate comprehensive care coordination initiatives, establishing protocols, driving process improvements, and managing resources to enhance patient care journeys and operational efficiency. You'll lead, mentor, and empower your team, fostering excellence, collaboration, and continuous improvement, while upholding quality, safety, and compliance.

To be successful, you'll combine strong clinical acumen with exceptional organizational and analytical skills, a proactive problem-solving approach, and the ability to inspire your team, transforming strategic visions into tangible, positive impacts.

  • The Supervisor directly oversees and adjusts, when necessary, staff scheduling to ensure appropriate personnel resources for departmental operations.
  • The Supervisor provides timely support, coaching, counseling or corrective intervention within scope of authority as indicated, maintaining documentation and engaging Human Resources and the department Manager/ Director when appropriate.
  • This position performs ongoing assessments and maintains supportive documentation of staff professionalism, productivity, compliance, communication and collaboration to assist with the development of annual performance evaluations.
  • The Supervisor performs ongoing assessment of staff educational, resource and equipment needs and collaborates with the department Manager/Director to secure needed resources.
  • The Supervisor guides Care Coordination activities according to the needs, requirements, and policies of the Medical Center, the affiliated medical groups and health plans, Dignity Health, any Federal and State agencies, and according to standard practices of the professions under the supervisor's accountability.
  • In addition, this position considers the population served by the medical center and area clinical integration programs and leads efforts to optimize care coordination across the care continuum. This coordination ensures a plan of care for patients in all stages of health needs.

Job Requirements

Required:

  • Bachelors Of Nursing or experience equivalent upon hire
  • Minimum of 3 years in a CareCoordination or Case Manager role
  • Registered Nurse: AZ upon hire 
  • Certified Case Manager or Accredited Case Manager upon hire
