Responsibilities

The incumbent provides care, treatment and service to Home Health and Hospice patients and their families in their place of residence within thier scope of practice. Functions as a member of the interdisciplinary team.

Job Duties:

  • Meets the discipline-specific productivity standard.
  • Demonstrates competency in the care of patients during field observation by the clinical supervisor.
  • Completes and submits documentation following established guidelines, as measured by defined evaluation processes (i.e. chart audits/QM reviews/etc.)
  • Physician orders are written according to policy and reviewed/co-signed by RN.
  • Documents follow-up of identified problems as clinically supervised by RN.
  • Reviews and documents clinical progress toward goals.
  • Demonstrates care as coordinated and supervised by RN and RN established Care Plan. Appropriate communication with other disciplines and documents that communication, as measured by defined evaluation/audit processes. (doc. communications, IDT notes, progress notes etc.).
  • Demonstrates compliance with agency processes to meet State regulatory and Federal Conditions of Participation requirements as measured by defined evaluation processes (i.e. U.R review, Patient Care, IDT processes etc.)
  • Documents appropriate management of visit utilization and authorization status as measured by a designated evaluation tool.
  • Performs care and other duties as directed/indicated.
  • Participates in on-call according to scope of practice as needed.
  • Support acceptance of referrals and intake.
  • Supports Telehealth monitoring under supervision of RN.


Qualifications

Minimum Requirements:

Education and Experience:

  • LVN Experience within the last 5 years.
  • 2 years recent experience as a LVN in a Home Health or Hospice Agency.

Licensure:

  • Currently licensed as a LVN in California.
  • Current California driver's license
  • Possession of automobile insurance and access to an operating motor vehicle.
  • Current Basic Life Support (BLS)
  • IV Certification preferred.
