Responsibilities

Physical Therapist – Community Hospital of San Bernardino

Join a compassionate and skilled rehabilitation team where your expertise as a Per Diem Physical Therapist will help patients regain function, independence, and confidence. At Community Hospital of San Bernardino, we are looking for a licensed professional to provide high-quality care across a range of medical conditions and patient populations.

As a valued member of our therapy team, you will evaluate and treat patients using evidence-based practices, craft individualized treatment plans, and collaborate with interdisciplinary teams to deliver patient-centered care. This role allows you to make a meaningful difference while maintaining flexible scheduling.

Key Responsibilities Include:

  • Perform comprehensive evaluations and establish personalized treatment goals
  • Administer therapeutic interventions in compliance with physician orders and professional standards
  • Educate patients and caregivers on home exercise programs and recovery expectations
  • Document patient progress accurately and timely in accordance with hospital policies
  • Participate in discharge planning and coordinate with care teams to ensure optimal outcomes
  • Provide guidance and oversight to support personnel and therapy students
  • Contribute to quality improvement efforts and department initiatives

Qualifications
  • Current registration, certification, or license as a Registered Physical Therapist by the respective state of which one practices
  • BLS - Basic Life Support certification is required.
  • Bachelors (BA, BS) degree in Physical Therapy is required.
