Responsibilities

Job Summary:

Provides hands-on direct patient care including evaluation, treatment, education, discharge planning and documentation per department standards as a member of the healthcare treatment plan. This would include coordinating and modifying patient’s care, treatment goals and discharge planning recommendations to achieve a maximum level of patient functional independence. Position also provides expertise in areas of manual therapy, lymphedema, hand therapy, informatics, sports medicine, emergency care or neurologic rehabilitation.

Essential Functions:

  • Interviews patient, family, caregivers, significant others to obtain information about prior level of
  • function, home environment, family relationships, health and psychosocial history.
  • Performs client tests, measurements and evaluations as indicated for the patient’s condition and as per
  • any established best practice guidelines and/or department care standards.
  • Establishes measurable/functional treatment goals that are realistic and consistent with
  • patient/family/caregiver needs.
  • Plans and prepares written treatment programs based on evaluation, age-related and diagnosis-
  • related policies. Administers treatments as prescribed in the treatment plan. Evaluates effectiveness of
  • treatment and adjusts the treatment to achieve maximum benefits.
  • Monitors all patients under his/her care: supervises Physical Therapist Assistants and/or Technicians
  • who are involved with treatment, and assures appropriate treatment and documentation; submits a
  • modified plan of care to the physician for altered plans of care; reports any adverse reactions to
  • treatment or changes in condition to the physician as indicated.
  • Institutes or assists with emergency measures for sudden adverse developments such as cardiac arrest or medical conditions requiring activation of a code, rapid response team, or activation of 911.
  • Implements and assesses appropriate patient and caregiver education per Plan of Care and phases of
  • progress.
  • Establishes and maintains collaborative communications with physicians, other healthcare
  • professionals, and facility departments. This would include team conferences, CCU rounds, other
  • patient care rounds or assigned meetings.
  • Consistently meets department productivity standards (90%) for outpatient and (75%) for inpatient.
  • Completes required reports, notes and other client-oriented documentation in a timely manner
  • according to policy and department standards, including G-code functional reporting and associated
  • modifiers.
  • Complies with changing regulations related to insurance, The Joint Commission, state/federal and other requirements as directed.
  • Documents medical necessity and skilled need for continued treatments in evaluations/re-evaluations
  • and progress notes.
  • Meets facility standard for Focus on Therapeutic Outcomes (FOTO) status and discharge surveys
  • (minimum of 10 completed per quarter). Uses FOTO recommendations appropriately or documents
  • reasons for exceptions (i.e. progress to date and expectation of significant progress if patient extended
  • beyond FOTO recommendations). (outpatient)
  • Enters charges accurately into Electronic Health Record or on appropriate charge slip.
  • Consistently participates with department communication processes: checking email daily, attending at least 80% of all scheduled meetings and in-services, and reviews meeting minutes if unable to attend.
  • Maintains specialized training requirements for a certification in Lymphadema (LANA), hand therapy
  • (CHT), manual therapy (FAAMOPT), Acute Care, Neurologic rehabilitation (APTA NCS or GCS and
  • specialty training in PNF, constraint induced movement therapy, vestibular stimulation or facilitation
  • therapies), emergency care or informatics (documented training in current EMR, FOTO, Meditech and
  • Redoc).
  • Provides public or other health care provider or staff training per year.
  • Participates in developing approaches for specialized services in area of expertise, in coordination with physician office to educate the community of specialty rehabilitation services.
  • Other duties as assigned by management.

Qualifications

Required Education and Experience:

  • Doctoral degree in Physical Therapy from an APTA accredited school of Physical Therapy

Required Licensure and Certifications:

  • Current license issued by the State of Oregon as a Physical Therapist or current PT compact privilege to practice in Oregon
  • AHA BLS certification within thirty (30) days of hire

Required Minimum Knowledge, Skills and Abilities:

  • Incumbents in this position must maintain a current knowledge of safety issues needed in the treatment of patients, especially regarding body substance precautions and body mechanics to protect self, patients, and personnel working with therapists.
  • The incumbent must abide by the ethical guidelines as outlined by the American Physical Therapy Association.
  • Ability to prioritize, organize, and use time management skills, problem solve, and productive communication.
  • Must possess a high degree of precision and accuracy.
  • Good written and oral English communication skills; excellent interpersonal skills.
