Manager of Community Health - Make a Difference!

Are you a passionate leader looking to make a real impact on a vibrant community? CHI St. Joseph is seeking a dynamic and dedicated Manager of Community Health to lead our program and champion the health and well-being of our community.

As the Manager of Community Health, you will be responsible for the overall direction, coordination, and evaluation of all programs and services administered by the department. This is a unique opportunity to shape the future of public health in Hubbard County and work collaboratively with North Country Community Health Services (CHS) to deliver essential services.

Key Responsibilities:

  • Strategic Leadership:Provide visionary leadership and strategic direction for the Department, ensuring alignment with the MN Public Health Statute and the needs of the community.
  • Program Development & Management:Plan, develop, implement, and direct a comprehensive range of public health programs and services for Hubbard County.
  • Financial Stewardship:Prepare and manage departmental budgets, oversee grant work plan and financial management, and ensure responsible allocation of resources.
  • Community Health Improvement:Identify community health needs, develop and implement Community Health Improvement Plans (CHIP), and actively participate in the CHS Strategic Plan and Quality Improvement programming.
  • Team Leadership:Supervise, mentor, and evaluate staff, fostering a positive and productive work environment. Manage work schedules, caseload assignments, and administer personnel policies.
  • Collaboration & Coordination:Coordinate public health services with other public and private agencies, serving as an advisor to the North Country CHS agency.
  • Emergency Preparedness:Coordinate the department's response efforts during emergencies, ensuring the safety and well-being of the community.
  • Community Engagement:Attend and participate in relevant public health meetings, provide educational trainings, and promote positive community awareness regarding available services and health prevention/promotion topics.
  • Performance Evaluation:Evaluate the quality and productivity of services and grants through data analysis, reports, and cost studies, working in conjunction with staff, grant managers, and the North Country CHS.
  • Policy Development & Review:Develop and review departmental policies to ensure compliance and effectiveness.
  • Outcomes Reporting:Ensure accurate and timely reporting of program outcomes and performance metrics.

Qualifications
  • Bachelor's degree in nursing required
  • Active MN RN License required
  • Public Health Experience Preferred
