We are seeking a dedicated and compassionate Community Health Worker (CHW) to join our team and contribute to meaningful change in people's lives.


About the Role:
As a Community Health Worker at Dignity Health East Valley, you will play a crucial role in facilitating access to local community resources for individuals (patients/participants) and, when necessary, their families, to foster healthier lifestyles. You will collaborate closely with community-based organizations and members to assess their health needs, offer education and support, and advocate for their access to healthcare services. Acting as a bridge between patients (participants/community members) and health/social service systems, you will work alongside other healthcare professionals to ensure the delivery of optimal care.

Job Duties:

  • Assist individuals and communities in adopting healthy behaviors, including addressing chronic diseases (e.g., diabetes, heart disease) and comorbidities (e.g., depression, arthritis)
  • Empower participants to achieve their health goals. This involves a collaborative approach, working alongside participants to identify their unique risks, needs, and barriers, and then developing a personalized action plan to address them one by one.
  • Facilitate and link participants (or families) with applications for social services like SNAP, Medicaid, etc.
  • Serve as a liaison, or as a bridge, between participants, healthcare providers, hospital staff, and community/state resources
  • Providing one on one coaching empowering individuals to achieve their health goals. Adapting the coaching style and approach to the individuals, learning preference, culture, background, and beliefs.
  • Demonstrate active listening by creating a safe and supportive space where individuals feel comfortable sharing their challenges and concerns. Listen attentively and demonstrate empathy for their experiences.
  • Proficient in electronic health records (EHR) navigation, strong at comprehending and analyzing content, and skille
  • Promote the importance of immunization (vaccines) and facilitating access to immunization services for both children and adults. Provide clear and accurate information about the importance of immunization for preventing serious diseases, protecting individuals and the community, and promoting overall health and well-being.
  • Interpret, translate, or provide cultural mediation related to health services or information for participants.

Qualifications

Reuirements:

Experience:

One (1) year of experience working with the community.
Experience working in a multicultural setting.
Knowledge of local community nonprofit agencies or organizations serving the vulnerable populations

Education:

HS Diplona/GED

Certified Community Health Worker or Complete Community Health Worker Certification within six (6) months of hire.
Basic computer skills required, electronic health record (EHR) experience.
Familiarity with Medicaid and other Government systems.

Licensure:

AZ Fingerprint Clearance

AZ DL

Special Skills:

Ability to work independently, set priorities and manage work within timeframes.
Demonstrates strong organizational skills and the ability to adapt swiftly to evolving priorities.
Ability to handle highly confidential information

Bilingual proficiency is desireable
