Responsibilities

Job Summary / Purpose


Provides a quality level of comprehensive clinical pharmaceutical services demonstrating sound judgment based upon knowledge and application of professional, departmental, and organizational principles and standards. Demonstrates an ability to coordinate subordinate activities by providing direction and guidance concerning the operational activities of the department. Has obtained Board Certification in a recognized field of pharmacy practice.

What You'll Do:

  • Clinical Documentation: Accurately and thoroughly document your clinical activities and interventions, including consults, patient education, and other patient-specific information, following established policies and procedures. Your documentation will contribute to a comprehensive patient record and support continuity of care.
  • Medication Order Interpretation & Verification: Skillfully interpret medication orders and prescriptions, identifying potential drug interactions and ensuring appropriate drug selection. You'll accurately enter and/or verify orders in the computer system, ensuring medications are labeled and dispensed in a timely manner.
  • Medication Safety & Accuracy: Verify and check medications prepared by other pharmacy personnel, ensuring accuracy and adherence to safety protocols. Your attention to detail will contribute to a safe medication use process.
  • Communication & Collaboration: Effectively communicate drug information to patients and healthcare professionals. You'll resolve drug-related problems by collaborating with other healthcare professionals to achieve optimal patient outcomes. You'll clarify unclear orders with prescribers and assist in appropriate drug selection and therapeutic regimens.
  • Continuous Learning & Development: Maintain a strong knowledge base to provide clinical services for patients of all age groups. You'll provide basic clinical services as required by policy and procedure, staying up-to-date with the latest advancements in pharmacy practice.
  • Technology Proficiency: Utilize and maintain a working knowledge of automated systems to efficiently manage medication-related processes.
  • Technician Leadership & Support: Provide guidance, direction, and support to pharmacy technicians, contributing to their professional development and ensuring efficient workflow.
  • Education & Training: Participate in the orientation, instruction, and training of pharmacy students, interns, pharmacy technicians, residents, pharmacists, and other healthcare professionals. Share your knowledge and expertise to help shape the future of pharmacy.
  • Residency Program Leadership (If Applicable): Serve as the Residency Program Director or Coordinator, providing leadership and guidance to the residency program (if applicable to your location).
  • Regulatory Compliance: Perform duties in accordance with the requirements of all regulatory and governing bodies, ensuring adherence to the highest standards of practice.
  • Continuing Education: Complete mandatory education requirements at the system and department levels, demonstrating your commitment to continuous professional development.
  • Patient Counseling & Education: Provide patient counseling, drug information, and documentation as appropriate, empowering patients to actively participate in their medication therapy.
  • Other Duties: Complete other duties as assigned within the scope of your licensure, contributing to the overall success of the pharmacy team.

Qualifications

Education/Experience:

  • Bachelor or Science in Pharmacy or Doctor of Pharmacy degree
  • 1 year of pharmacist experience + certification

License/Certification:

  • BLS-CPR
  • Active State Pharmacist License in good standing in the state in which the employee works: PHARM:XX
  • Board Certification in a Pharmacy Specialty, based on the specialty title under which the employee works

    • BCCP for Cardiology Pharmacy
    • BCSCP for Compounded Sterile Preparations Pharmacy
    • BCCCP for Critical Care Pharmacy
