Responsibilities

The social worker is a critical member of the multidisciplinary team. As our Licensed Clinical Social Worker at Marian Regional Medical Center  you will primarily be supporting our Pacific Health Centers of the Central Coast clinic locations. The LCSW will help patients and families address psychosocial needs that may be impacting their health and well-being so they have a care plan that fully incorporates those needs. 

Every day you will serve as the psychosocial expert on the multidisciplinary care team. You will be expected to perform bio-psycho-social spiritual assessments, micro and mezzo level clinical care planning, and enroll and treat patients in the appropriate clinical programming. 

To be successful in this role, you must  utilize a clinical skill set to work with patients, families, and the care team to comprehensively address psychosocial needs in a trauma informed manner. 

This is a hybrid role.

  • Assessment, clinical care planning, and disposition planning for patients and families with identified psychosocial needs
  • Work with the multidisciplinary team to ensure that the psychosocial needs of the patients are being incorporated into the treatment plan in a trauma informed manner. 
  • Provide advocacy and evidenced based clinical care for our most vulnerable patients and community members across the care continuum.
  • Opportunity to provide clinical supervision to social workers registered with the Board of Behavioral Sciences as an Associate Clinical Social Worker.

Qualifications

Minimum Experience Required

At least two years of clinical and/or medical social work experience required.

Education Required

Master's Degree from a School of Social work accredited by the Council of Social Work Education

Licensure Required

Licensed Clinical Social Worker or Licensed Clinical Social Worker in process preferred in California; LCSW or LMSW preferred AZ.

Special Skills

Knowledge of contemporary behavioral and systems theories relevant to health care; end-of-life dynamics and interventions; grief and bereavement
counseling; substance abuse identification and interventions; support for victims of abuse neglect or violence; community or financial resources for
underserved or vulnerable populations. SSDI SDI and other governmental/county requirements/programs. Thorough comprehension of OBRA and Title 22 is required.
