The posted compensation range of $55.55 - $79.06 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Job Summary and Responsibilities

Are you a passionate and dedicated Registered Nurse looking to truly impact patients' lives? As an RN Care Coordinator, you'll be the driving force behind ensuring seamless care transitions and optimal outcomes for our patients. You'll be the advocate, the navigator, and the compassionate guide, working collaboratively with a multidisciplinary team to meet each patient's unique needs. In this vital role, you'll oversee the entire care journey, from admission to discharge, ensuring quality, cost-effective care that aligns with hospital standards and regulations. You'll be a master communicator, collaborating with physicians, nurses, insurers, and post-acute providers to progress care and achieve the best possible results.

  • Clinical Expertise: Ability to apply clinical guidelines to ensure appropriate progression of care.
  • Critical Thinking & Problem-Solving: Demonstrated ability to analyze complex situations and develop effective solutions.
  • Collaboration & Communication: Exceptional interpersonal, written, and presentation skills to effectively collaborate with diverse stakeholders.
  • Utilization Management & Case Management: Understanding of how utilization management and case management programs integrate to optimize patient care.
  • Teamwork & Adaptability: Proven ability to thrive in a fast-paced, self-directed environment, working collaboratively as a team player.
  • Regulatory Knowledge: Familiarity with CMS standards and requirements.
  • Organization & Time Management: Highly organized with excellent time management skills, including the ability to prioritize and delegate effectively.
  • Technical Skills: Ability to pass annual Inter-rater reliability test for Utilization Review product(s) used.
  • Customer Service: A commitment to providing excellent customer service.

Job Requirements

Required 

  • CA Registered Nurse (RN) License required.
  • Minimum of 3 years nursing experience in an acute hospital setting.
  • BSN degree or experience equivalent preferred
  • CCM certification preferred.
  • Excellent customer service and presentation skills are a must. Strong interpersonal and written communication skills are essential. Demonstrated ability to apply analytical and problem solving skills.
  • Demonstrated ability to manage multiple tasks or projects effectively. Ability to work independently as needed with a high degree of detail orientation.
  • Masters Degree in nursing with a concentration in Case Management can serve as a substitute for the experience requirement.
  • If operational conditions permit, training a candidate without the required experience may be considered.
  • Prior Care Coordination experience in a clinical or insurance setting is required.
  • Ability to work efficiently in a fast-paced environment with changing priorities.
  • Must maintain current Fire Safety cards.
  • Relevant certification is preferred (CCM, ABQAURP or ACM).
