- Associates degree from an accredited school approved by the American Occupational Therapy Association.
- Current license to practice in the state serviced.
- Must obtain and maintain current CPR Certification
- One-year occupational therapy experience required.
- Must obtain a National Provider Identifier (NPI) as this is now a requirement in step with the Health Insurance Portability and Accountability Act (HIPAA)
- Home care experience preferred.
