Responsibilities
JOB SUMMARY:
This job is responsible for leading clinic-based quality improvement initiatives and projects across the Virginia Mason Franciscan Health (VMFH) ambulatory environment/clinics. An incumbent makes best practice recommendations, identifies improvement opportunities, develops project plans, forms work teams and leads clinic-level initiatives to improve the quality of care being delivered in the ambulatory setting. Work also includes facilitating the gap closure of health care needs for VMFH our patients, particularly in terms of prevention and chronic disease management.
Work focuses on all aspects of quality process improvement and is conducted in accordance with professional standards, organizational objectives and applicable regulatory/accreditation requirements. The intended outcomes are to create and implement work plans to help achieve organizational ambulatory quality strategies, including improvement in performance rates on topics such as preventative wellness visits, cancer screenings, hypertension control, diabetes management and overall value-based revenue capture.
Work also includes coordinating various multi-disciplinary teams, providing support, expertise and guidance in: 1) the application of accepted clinical quality improvement methodologies/tools; 2) reviewing patient gap list data and providing improvement strategies; 3) risk assessment and outcomes measurement; 4) evidence-based best practices research; 5) documenting and implementing process improvements; 6) leading specific improvement strategies for a particular metric or area of focus; and 7) evaluating desired vs. actual clinical effectiveness of performance improvement efforts
Qualifications
Education/Work Experience Requirements
Bachelor’s degree in healthcare administration, nursing or other healthcare-related field, and five years of progressively responsible related work experience that would demonstrate attainment of the requisite job knowledge.
A minimum of three years in a lead or management role is strongly preferred.
Master’s degree in a healthcare discipline is strongly preferred.
Licensure/Certification
- Current licensure as a Registered Nurse by the Washington State Board of Nursing.
- Certification as a Certified Professional in Healthcare Quality (CPHQ) is strongly preferred.
