Hospice care involves a team-oriented approach to expert medical care, symptom and pain management, and emotional and spiritual support expressly tailored to the patient's needs.  Our primary goal is to fulfill our patient's wishes and to support their families and caregivers. Hospice can help the patient maintain the highest quality of life possible. We strive to care for the whole person, not just the disease.

Dignity Health Hospice also provides bereavement services for a full year after a loss. Our trained bereavement team offers support to help families and caregivers cope with the loss of a loved one.


Responsibilities

About This Position

In keeping with the Marian Medical philosophy the Pharmacist is responsible for the procuring manufacturing compounding dispensing distributing storing and administration of drugs biologicals and chemicals according to standard practices of the professional State and Federal laws and regulations and Departmental policies and procedures.

Responsible for provision of drug information to other health professionals patients and their families and the public. Ability to exercise sound and professional judgement in the performance of the job functions. Ability to communicate effectively both orally and in writing. Strong attention to detail. Ability to analyze complex tasks systems problems and design and implement the most effective solution. Ability to work tactfully and effectively with team members the patient and their family and the public. Comprehensive knowledge of the practices principles methods and equipment utilized in providing Pharmaceutical services. Ability to organize and prioritize work. Current and valid California license to practice pharmacy.


Qualifications

Minimum Qualifications:

  • One (1) year of acute care hospital pharmacy or home infusion therapy.
  • Current and valid California license to practice pharmacy.

Preferred Qualifications:

  • Pharm.D. Degree preferred.
  • Experience in I.V. admixture unit dose distribution
  • Pharmacokinetics dosing
  • Pharmacy intervention and medication usage evaluation.
