Our comprehensive services cater to patients recently discharged from the hospital or homebound individuals requiring professional nursing and/or rehabilitative care. We take pride in our unique offerings, including Chronic Disease Programs and Specialty Care, the Largest Rehabilitation Team on the Central Coast, the Largest Wound Care Team on the Central Coast, Maternal and Infant Services, Medical Social Workers, Nutritional Counseling, Pediatric & Neonatal Care, and Skilled Nursing. 


Responsibilities

As a supervisor, you will play a crucial role in our hospital's mission to serve as a community health service provider. Your collaboration with other area supervisors will be instrumental in achieving this goal. Additionally, as a supervisor, you'll: 

  • Assist in implementing a team Quality Management Program consistent with the hospital-wide Performance Improvement Plan. 
  • Drive metrics (KPI) and develop action plans to meet expected goals. 
  • Ensure that the staff is well-trained in the services offered, playing a critical role in providing quality services, thereby contributing to our patients' well-being and our organization's success.
  • Address escalated issues or resolve team issues, including problem resolution related to customer complaints, concerns, and suggestions.

This position would take two days in the office and three days of remote work.

The ideal candidate will reside in San Luis ObispoCounty, California. We can also offer relocation (if applicable), making the transition to this rewarding position even easier. We will offer a 10% sign-on bonus as a token of our appreciation


Qualifications

Education and Experience:

  • Minimum of two (2) years of management experience (within the last five years) in a home health agency primary care clinic or health facility.
  • Sufficient background knowledge and expertise in clinical decision-making for the patient population assigned.
  • Preferred candidates will have recent Home Health experience and familiar with HomeCare Homebase EMR.

Licensure:

  • A Registered Nursing License (RN) license in the state of practice (California) is required.
