Responsibilities

The Service Coordinator - Social Worker under the supervision of the Director of Nursing and Administrator performs social service functions within the adult/sub acute unit. Serves as member of the interdisciplinary team liaison to advocacy agencies and support resource to residents families and peers. Assures compliance with the state and federal regulatory and accrediting agencies such as California Department of Public Health Title 22 and OBRA. This position requires the full understanding and active participation in fulfilling the Mission of Community Hospital of San Bernardino. It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support Community Hospital of San Bernardino strategic plan and the goals and direction of the quality improvement process and activities.

Qualifications
    • Experience working in a subacute facility or related health care facility environment REQUIRED. 

    • BSW in Social Work or BA/BS in health sciences is required.

    • Knowledge of the growth and development process and ability to affect age appropriate interventions..
    • Knowledge and skills necessary to provide services appropriate to the age of the patients served.
    • A working knowledge of basic services provided by various health early intervention and welfare organizations serving children in the community is required.
    • Basic Life Support (BLS) certification required.
