Responsibilities

The Clinical Document Specialist - Case Manager is responsible for managing a continuum of care from admission through discharge for assigned patients. The role reflects appropriate knowledge of RN scope of practice, current state requirements, CMS Conditions of Participation, EMTALA, The Patient Bill of Rights, AB1203 and other Federal or State regulatory agency requirements specific to Utilization Review and Discharge Planning. The Case Manager partners with the medical staff, utilizes scientific evidence for best practices, relevant data and compliance with the mission/philosophy, standards, goals and core values of Dignity Health DIGNITY HEALTH (dignity, collaboration, justice, stewardship and excellence). Primary elements of the case manager role include: Care Coordination, Utilization Review and Discharge Planning. This position requires the full understanding and active participation in fulfilling the Mission of Community Hospital of San Bernardino. It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support Community Hospital of San Bernardino's strategic plan and the goals and direction of the quality improvement/process improvement activities.


Qualifications

Requiremetns:

  1. Minimum of three (3) years in acute care hospital experience required. A Masters degree in Nursing (MSN) with a concentration in Case Management can serve as a substitute for the experience requirement.
  2. Current Registered Nurse (RN) License to practice professional nursing within the state employed required.
  3. Graduate of an Registered Nurses (RN) program with a Bachelor of Science in Nursing (BSN) or MSN required. If a Case Management position has been posted for 30 days and no qualified candidates have applied, the Case Management department may interview and hire licensed Registered Nurses matriculating in a national or state approved BSN or MSN program.
  • Excellent verbal and written skills required.
  • Must read, speak and write English fluently.
  • Excellent organizational skills required.
  • Familiarity with the following: ANA Nursing: Scope and Standards of practice, ANA Code of Ethics for Nurses with Interpretive Statement, Nurse Practice Act for state of employment Core Measures
  • Basic understanding of desk top computers and work Related software required.
