Responsibilities

**Current and valid California license in one of the rehabilitative therapies Required**

  • Physical Therapy
  • Occupational Therapy 
  • Speech Therapy

In keeping with the Dignity Health Core Values, the Rehabilitation Admission and Discharge Coordinator will assist the manager with the admission and discharge processes of the Acute Rehabilitation Center. The Coordinator facilitates timely pre-admission assessments for patients referred to rehab; ensures timely documentation and communication regarding pre-admission assessments to referring physicians, acute case managers, acute nursing and rehab clinical team; ensures accurate insurance verification and authorization prior to patient admission; provides patient and family with information regarding the Rehabilitation Center prior to admission; ensures each rehabilitation patient receives discharge planning services with appropriate referrals and arrangements for DME prior to discharge; and assists the Admission and Discharge.


Qualifications

Experience:

  • 3 years of clinical experience in acute rehabilitation setting.
  • Previous Case Management and working with third party payors is required.

License/ Certifications:

  • Current and valid California license in one of the rehabilitative therapies;
    • Physical Therapy
    • Occupational Therapy 
    • Speech Therapy
