Responsibilities

Shift: 8am - 4:30pm

Core Schedule: Tuesday - Friday

Job Summary:

The Palliative Care RN provides advanced care assessments, patient case management, palliative care services and related services to patients and families. The Palliative Care RN is responsible for the coordination of services and acts as the liaison between healthcare providers and patients.

Essential Functions:

  • Collaborates with nursing interdisciplinary team and nursing directors to provide palliative care services to patients and their families.
  • Serves as a clinical expert for palliative care. Works closely with the Palliative Care Physician Champion and other physicians to develop relationships that promote understanding of the advanced medical model represented by Palliative Care services.
  • Determines reason for consults (i.e. goals of care discussion, family conference, pain/symptoms management, etc.) and performs medical record review.
  • Performs a bedside palliative care patient/family assessment/evaluation meeting with or without Physician. Assists to answer questions about Advance Directives, Physician Orders for Life-Sustaining Treatment (POLST) and comfort care.
  • Identifies and communicates with other disciplines required to develop plan of care as appropriate.
  • Discusses and documents psychosocial assessment, impressions and recommendations with attending/consulting physicians/staff as indicated.
  • With physician consent, discusses plan of care with patient/family on an ongoing basis throughout hospitalization.
  • Conducts or attends Family Conferences and facilitates interaction between patient and family.
  • Communicates with patients and family members experiencing the grieving and bereavement process, in a caring manner.  Collaborates with Chaplain on “after-death” support and events
  • Identifies eligibility for private and public sector funding for services (e.g. Medicare, Medicaid, community resources, and charitable funds).
  • Assesses the patient’s social support system and relationships (e.g. family, friends, significant others, community groups).
  • Reviews information about the patient’s social and financial resources (e.g. income, living situation, and insurance).
  • Makes recommendations to discharge planners for inpatients.
  • Assists with pain management, symptoms management and end of life planning.
  • Assists with maintaining Joint Commission accreditation standards. 
  • Participates and assists in any research, quality improvement and other studies involving palliative care. 
  • Maintains clinical skills through participation in appropriate continuing education programs and self-learning activities.
  • Provides materials for advancement of learning about Palliative Care services.
  • Participates in Palliative Care quality improvement measures.

Qualifications

License/Certification:

  • Current RN license issued by the state of Oregon.
  • Must have current BLS upon hire.
  • Certification in Hospice or Palliative Care preferred.

Experience:

  • 2 years RN experience in an acute care setting required.
  • Palliative Care experience required. Acute care Palliative Care experience preferred.
  • Must be able to attend End-of-Life Nursing Education Consortium (ELNEC) and Center to Advance Palliative Care (CAPC) program within two years.

Education:

  • Associates Degree in Nursing required. BSN preferred.
  • BSN preferred.
