Responsibilities

The Clinical Pharmacy Specialist provides clinical direction and leadership to clinical staff and staff pharmacist. Actively participates in formulary management pharmacotherapy services and drug therapy management.

The decentralized Hematology/Oncology Clinical Specialist is responsible for the delivery and documentation of pharmaceutical care provided to patients throughout the medication use process (selecting, prescribing, dispensing, administration and monitoring of medications). The Clinical Specialist provides clinical direction for the pharmacy and acts as a pharmacy liaison. The pharmacist in this position actively participates in formulary management, pharmacotherapy services, and drug therapy management.
This position is critical to the success of Dignity Health St. Joseph's Hospital and Medical Center and requires the full understanding and active participation in fulfilling the Mission of Dignity Health.


The Inpatient Hematology/Oncology Pharmacist:

  • Performs order verification and completes kinetics consults for associated hospital areas
  • Provides direct patient care
  • Acts as pharmaceutical consultant to provide drug information to physicians and nurses
  • Precepts pharmacy practice residents and pharmacy students
  • Drives pharmacy initiative development and implementation, and provides continued oversight
  • Acts as a pharmacy representative in clinical, quality, and regulatory activities pertaining to optimal medication use
  • Contributes to P&T Committee activities as appropriate
  • Actively participates on patient care rounds with providers in a collaborative practice model
  • Provides comprehensive pharmacotherapy services at the top of her/his license

Qualifications

Requirements:
Graduate degree in pharmacy
Current Arizona pharmacist license in good standing
PGY1 pharmacy residency or practice equivalent

Specialized residency training or three years hospital pharmacy experience with >50% of time spent providing direct patient care

AZ Pharmacy license Board Certified Pharmacotherapy Specialist preferred


Preferred Qualifications:
PGY2 residency in oncology
Two or more years of acute clinical pharmacy oncology experience highly preferred
BPS Board Certification in Oncology (BCOP)
Strong clinical pharmacy skills
Experience in precepting both pharmacy residents within an ASHP accredited PGY1 pharmacy residency program and APPE students from ACPE accredited Colleges of Pharmacy

Experience with Cerner Millennium
