The posted compensation range of $33.84 - $50.34 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.

Job Summary and Responsibilities

As a Navigator RN at CommonSpirit Health, you will serve as a vital member of our healthcare team, working collaboratively with physicians and staff to provide a comprehensive "medical home" and seamless care coordination across the healthcare continuum. 

Every day, you will ensure patients receive the highest standard of care by monitoring clinical quality metrics, upholding departmental policies, and championing evidence-based best practices to improve patient outcomes.

By maintaining accountability for high-quality standards and facilitating proactive care, you will play an integral role in sustaining our commitment to compassionate, patient-centered care.


Disease Registry & Quality Oversight

  • Maintain the accuracy and currency of the disease registry database.
  • Monitor patient compliance by identifying individuals overdue for visits, labs, or referrals and coordinating necessary follow-up care.
  • Analyze clinical metrics (e.g., blood pressure, glucose levels) to identify patients not meeting goals and initiate follow-up interventions per established protocols.
  • Generate comprehensive quality performance reports at the patient, physician, and clinic levels.

Pre-Visit Planning & Clinical Preparation

  • Conduct thorough pre-visit chart reviews to identify required preventative health maintenance, immunizations, and chronic disease interventions.
  • Execute clinical interventions prior to provider appointments when authorized by standing orders.
  • Prepare and initiate office visit forms and pre-visit documentation to streamline provider workflows and improve care communication.

Patient Education & Self-Management Support

  • Collaborate with patients and families to establish actionable short- and long-term chronic disease self-management goals.
  • Assess and address medication adherence barriers for patients who are not meeting clinical outcome targets.
  • Utilize the 5A’s framework to partner with patients in developing sustainable health behavior change plans.

Professional Development & Administrative Standards

  • Contribute to the continuous improvement of clinical practices by integrating evidence-based guidelines and standardized protocols.
  • Ensure all clinical documentation is recorded accurately, concisely, and in a timely manner using required formats.
  • Participate in scheduled staff meetings to support ongoing information sharing and professional development activities.

Job Requirements

Required

  • Associate Of Nursing Associate Degree in Nursing (ADN)
  • Minimum of four (4) years experience as a RN
  • Registered Nurse issued by the state of Minnesota 
  • Basic Life Support (BLS) from the American Heart Association (AHA)


Preferred

  • Bachelors Of Science Degree in Nursing (BSN)

CHI St. Joseph’s Health of Park Rapids, part of CommonSpirit Health, is an award-winning, 25-bed critical access hospital located in the heart of lake country. Fully accredited by The Joint Commission, we offer a full range of services including 24/7/365 Emergency Department and Level IV Trauma Center, hospitalist program, testing, surgery, orthopedics, rehabilitation, and more so that you can experience better health.
