Social Worker - Care Coordination

St. John's Regional Medical Center

We are seeking a dedicated Care Coordination Social Worker to join our team and help improve the well-being of our patients and clients. As a Care Coordination Social Worker, you will play a vital role in ensuring that individuals receive comprehensive and coordinated care services.

Responsibilities
  • Conduct comprehensive assessments of patients' social, emotional, and environmental needs.
  • Develop and implement care plans in collaboration with healthcare providers, patients, and their families.
  • Coordinate and facilitate access to medical, social, and community resources.
  • Advocate for patients' rights and assist with navigating the healthcare system.
  • Provide counseling and support to patients and their families.
  • Monitor and track patients' progress and make necessary adjustments to care plans.
  • Collaborate with interdisciplinary teams to ensure a holistic approach to patient care.
  • Maintain accurate and up-to-date patient records and documentation.

Qualifications
  • At least one year of clinical and/or medical social work experience required.
  • Master's Degree from a School of Social Work accredited by the Council of Social Work Education.
  • No Licensure required.

Special Skills:

  • Knowledge of contemporary behavioral and systems theories relevant to health care; end-of-life dynamics and interventions; grief and bereavement counseling; substance abuse identification and interventions; support for victims of abuse neglect or violence; community or financial resources for underserved or vulnerable populations.
  • SSDI SDI and other governmental/county requirements/programs.
  • Thorough comprehension of OBRA and Title 22 is required in California.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Knowledge of healthcare systems, community resources, and social service programs.
  • Compassion, empathy, and a commitment to improving the well-being of others.

Please be advised that this location requires quarterly American Heart Association (AHA) Resuscitation Quality Improvement (RQI) competency certification post-hire.  This ongoing competency is required through resources internal to the facility; outside RQI competency resources are not acceptable for CommonSpirit Health compliance requirements.

This position is represented by SEIU-UHW and is covered by the terms and conditions of the applicable collective bargaining agreement.
