Responsibilities

The Social Work Case Manager is responsible for assessing the psychosocial needs of persons living with HIV/AIDS and developing implementing monitoring and evaluating an agreed upon individualized plan of care for each client. This positon is responsible for informing clients about and referring them to appropriate community resources and advocating for services. Through intervention and education the Social Work Case Manager supports the client in maintaining the highest degree of personal control and independence.


Qualifications

Minimum Requirements:

  • One year of HIV/AIDS related clinical experience  and knowledge of HIV/AIDS community resource preferred.
  • Spanish speaking ability desirable
  • Master's Degree from an accredited school of Social Work
  • Experience directly supporting LGBTQ street-involved (homeless) and/or active-drug using populations.
  • Must be able to communicate with others to gather information establish and maintain filing system use computers for scheduling and payroll management and use phone fax and personal computer
  • Passenger transport and customer service experience. 
  • Drug screening required prior to employment
  • Current Driver's license required, with a record showing no moving violations or DUI convictions in the last three years.
  • AHA BLS card
