Overview

CHI St. Alexius Health Garrison includes the services of a 22-bed critical access hospital, a 26-bed skilled nursing facility, an attached rural health clinic, and a newly remodeled Emergency Department that is available 24 hours a day. We offer many outpatient services, including physical therapy, radiology services, laboratory services, cardiac rehabilitation, and IV treatments.

Since opening in 1952, CHI St. Alexius Health Garrison has been dedicated to serving the residents of Garrison and the surrounding rural communities.

Garrison Memorial Hospital is equipped with 4 emergency rooms, on-site lab, radiology, CT and physical therapy services. Our patients have access to 24/7 emergency care, swing-bed program, and acute care services. Responsibilities

Job Summary / Purpose

To provide medically related social services to the residents of Garrison Memorial Hospital Nursing Facility and Swing Bed.  Social services are provided to ensure that each resident attain and maintains their highest level of physical, mental and psycho-social functioning.

Essential Key Job Responsibilities

  • Referrals/Applications: collect necessary data on all persons applying for admission to nursing facility, swing bed, respite and adult day care.  
  • Review information with appropriate staff to assess ability to handle the care and meet needs.  Maintain waiting list.
  • Resident Assessment and Documentation: Complete resident assessments, MDS and progress notes in a timely manner.  
  • Participate in the interdisciplinary care plan meetings.
  • Resident Advocate: provide required information about facility policies, procedures, etc. to residents and families on admission and thereafter as required.  
  • Offer social and emotional support to residents and families.  
  • Handle grievances in appropriate manner.  Promote residents rights.
  • Discharge Planning: Provide discharge planning for all discharges/transfers from swing bed and nursing facility.  
  • Complete notices, provide documentation, make referrals and arrange transportation.
  • Assist with scheduling appointments and arranging for transportation for residents as needed.
  • Organizational Development: Attend in-services/meetings as follows: Quality Improvement, Utilization Review, Department Head, Care Conferences.  
  • Involved in hospital, community and professional organizations and activities.  
  • Provide in-services and education to intra-hospital and community groups.
  • Works in conjunction with consulting Licensed Social Worker.  Follow the consulting Licensed Social Worker recommendations and guidance.
  • Performs other duties as assigned

Qualifications

High School Diploma

Must be able to relate very well with staff, patients, families, physicians, and other health care professionals. Must be able to maintain confidentiality. Must be able to communicate, maintain records and develop reports. Must be able to use a computer, keyboard and telephone.

Bachelor’s degree in Human Services or related field is preferred. Minimum of one year of experience in a health care setting working directly with individuals is preferred. Experience working in a hospital is desirable.
