Responsibilities

The Project Manager for Community Health and Housing is supporting the design, implementation and evaluation of programs across CommonSpirit Health that address housing and homelessness. The position is focused on managing the data and evaluation to support system and local efforts to better serve our populations experiencing housing insecurities and homelessness. This position will also support the expansion of the Research, Equity and Advisory Council on Housing Insecurities (REACHI).

The Project Manager for Community Health and Housing interacts with community partners and supports housing and homelessness related projects nationally to establish shared metrics and identify opportunities to scale best practices. The project manager works across multidisciplinary teams including care coordination, nursing, mission, and service area/facility based community health leadership. This position supports work to set priorities, develop strategies and initiatives, and build community based partnerships to improve homeless services capacity including housing, supportive services such as transitions of care, emergency resources and areas of prevention.

  • Supports the system wide strategy to address housing insecurities and homelessness prevention across our CommonSpirit footprint
  • Develops measurement and evaluation of strategies that demonstrates collective impact
  • Ensures the timely and accurate reporting of expense and program data and maintain grant compliance and reporting, including outcome measurement and grant budgets
  • Supports activities and expansion efforts related to the lived expertise council
  • Collaborates with cross-functional departments to align community health work with other relevant initiatives and organization priorities
  • Supports the development of an online grants platform
  • Supports the development of best practice resources, tools, trainings, and guidance on serving individuals experiencing homelessness or housing insecurity
  • Evaluates opportunities and takes actions to build strategic relationships to help achieve business goa
    Tracks outcomes and metrics for HHI programs, supports process improvement activities and prepares evaluations and reports on progress including data analytics
  • Research best practices to inform program evaluation, and sustainability efforts
  • Engages with CBOs and public entities that provide housing/homelessness services and stakeholders related to these programs including identification, conducting stakeholder meetings and trainings, and developing and implementing programs, and program oversight
  • Participates in SDoH sector learning and collaboration. Investigates issues, trends, and opportunities to advance health equity and address SDoH, with a focus on housing, both externally with partners and internally with other departments. Develops recommendations relative to identified issues, trends and opportunities

Qualifications
  • Bachelor's Degree in public health, health administration, finance or related fields
  • Minimum of 2-4 years experience working in a hospital, health agency or nonprofit community service organization
  • Project management experience required
  • Experience in homeless services
