Job Summary / Purpose


The Virtually Integrated Care (VIC) Care Transformation Nurse (as assigned within the regions) is a 50/50 hybrid role that works as the catalyst for patient centered care by leading a well coordinated care team and also fulfills the role of Virtually Integrated Care (VIC) RN as needed during implementation in assigned markets. While the VIC RN works virtually, they guide the bedside team on the plan of care and prioritization of care. The VIC RN is at the center of innovation and a transformational, evidence based practice care model. The experienced RN in this role will anticipate and direct the care of the patient through evaluating health promotion, risk reduction and clinical decision making. This role assumes accountability for client care outcomes through the assimilation and application of evidence based practice to design, implement, and evaluate client plans of care in a virtual environment setting. This role functions as the center of coordination and communication for a multidisciplinary team. A high degree of professional competence and autonomy will be demonstrated in performing the roles of 1) clinician, 2) educator, 3) consultant/collaborator, 4) leader. The VIC RN will function as a dynamic change leader for this transformational care model. The role will assist with direct staffing in virtually integrated care models in facilities across the CommonSpirit enterprise. The role will also assist with education and orientation as an integral part of the implementation team. This role will function as support for standardization and sustainment of critical virtually integrated care workflows. This individual will need to be flexible with their own schedule to align with the staffing needs of the market and implementation plan.

Essential Functions:

  • Approximately 50% of this role will be in direct staffing of the VIC RN role to provide staffing support during an implementation or expansion within a determined market. This will include travel to the hospitals that implementation or expansion is occurring in.
  • The VIC RN will lead the team by carrying out the following key job responsibilities:
  • Collaborates virtually with physicians, nurses and other health care team members in the provision of clinical care for the patient.
  • Assesses virtually the medical, physical, and psychological status of patients by reviewing patient records, obtaining a comprehensive problem-oriented health history and monitoring and evaluating laboratory, x-ray, EKG, and other diagnostic studies as medically indicated.
  • Interprets available data in order to establish a primary plan of care. The plan is developed through an interdisciplinary team approach and through multi-dimensional patient care conferences, ongoing patient assessments, and re-evaluation of patient problems or needs.
  • Responds to routine, urgent and emergent patient problems appropriately by demonstrating effective decisionmaking and provides follow-up to ensure that patient problems are resolved.
  • Effectively communicates with members of the bedside interdisciplinary team in a collegial manner to provide safe and error free care during routine and emergent conditions.
  • Serves as an advocate on behalf of patients and their families in regards to their cultural, spiritual and care needs.
  • Prioritizes and effectively handles multiple electronic devices and software applications in a professional and courteous manner to include the ability to manage data and create reports electronically.
  • Coaches and mentors nurses and nursing support on the floor around the care of the patient.
  • Virtually educates the patient and families on disease process, procedures and self-care to maximize the level of function and wellness upon discharge.
  • Focuses on exceeding national benchmarks for quality care delivery and resource utilization for cost containment and length of stay indicators.
  • Participates in achieving nursing division goals and supports a shared leadership structure. Participates in achieving organizational goals.
  • Assures appropriate level of understanding, awareness and compliance with all applicable TJC, federal, state and agency laws, regulations, guidelines and professional standards.
  • Through a defined reporting process, reports clinical, administrative, or technical problems that result in medical errors, unsafe care, or sub-optimal communication between appropriate health care professionals.
  • Participates in and initiates safety, security and infection control programs that are mandatory as well as those required and provided by the department.
  • Demonstrates work practices consistent with CommonSpirit Health and department-specific safety, security and infection control policies.
  • Facilitates a collaborative spirit assuring teamwork by providing direction to staff.
  • Establishes a relationship with the care team to help coach, mentor, and set expectations as needed.
  • Monitors and provides feedback on performance of employees as requested by the Region Manager.
  • May utilize delegation skills, but maintains accountability for identifying patient information to ensure improvement of care and patient outcomes.
  • Approximately 50% of this role will be focused on assisting with Care Transformation efforts such as orientation, mentorship and support of the other local VIC RNs and leading simulation and education of frontline staff functioning in the care model. They will serve as a virtual program leader monitoring workflows, technology and operational management.
  • Serves as an extension of our National leadership structure on shifts to expand leadership support.
  • Lead implementation program strategies as determined by program project plan
  • Function as leadership support and triage for any barriers that arise
  • Troubleshooting and critical thinking assessment prior to pulling in Regional Managers off hours.
  • Lead quarterly and as needed mentorship check-ins with local VIC RNs.
  • Provide expert direct support to promote workflow trials and advancement as ideas arise and promote small tests of change
  • Monitoring of Census and guide the teams on staff adjustment according to staffing grids
  • Assist with adjustment of assignments and creative staffing solutions

Qualifications

Education and Experience:

  • Bachelors Of Nursing upon hire
  • 5 years of overall acute care nursing experience Required
  • 3 years of nursing preceptor experience Required 
  • 2 years in a nursing leadership role Required 
  • 1 year experience with virtual care models Preferred
  • Advanced Practice Nurse experience may be considered Preferred
