Responsibilities
The Case Coordination RN/ Nurse Navigator will function both within the Cardiovascular Navigation Team and as a member of the Multidisciplinary Team (MDT) within the care center provide triage services to patients and support the clinical needs through direct patient care and pre and post op support. Through close communication among the Cardiovascular Service Line (CVSL) and both internal and external providers patient safety and care will be better identified and coordinated. The result will be an improved patient experience and outcomes. Local Travel up to 50% of time.
Responsibilities may include:
- Responsible for triaging incoming calls from patients assessing needs giving appropriate clinical dispositions base on established protocols.
- Facilitates access to primary care providers specialists healthcare facilities and community resources.
- Provides education to patients regarding immediate care advice and preventative healthcare maintenance.
- Adapts communication style to patients representing diverse personal professional cultural and socio-economic backgrounds.
- Ability to multi-task utilizing critical thinking verbal and listening communication skills while performing data entry.
- Utilizes computer database to assess educate and document all information.
- Documents all encounters according to departmental standards for medical legal and statistical purposes.
- Participates in quality improvement outcomes measurements process data collection and analysis.
- Meets minimum established quality and productivity standards.
- Mentors and provides leadership to support staff.
- Other duties as assigned.
Qualifications
Minimum Qualifications:
- Minimum 3 years of clinical experience in acute care or ambulatory care medical clinic required. Minimum 6 months of telephone/ call center/contact center triage nurse experience required.
- High School diploma or equivalent. Satisfactory completion of a formal Registered Nurse program pursuant to the Division of Allied Health Professions or military training that is equivalent to an accredited Registered Nurse program (determination by Dignity Health Medical Foundation H.R. department in conjunction with the State Division of Allied Health Professions.)
- Clear and current CA RN license
- BLS through American Heart Association.
- ACLS certified (or obtain certification within six (6) months of hire)
- Excellent interpersonal organizational and customer service skills are essential.
- Medical terminology familiarity with CPT and ICD coding procedures and reference tools and pharmacology appropriate to the Registered Nurse scope of practice is essential.
Preferred Qualifications:
- One or more (1+) years of telephone/ call center triage nurse experience preferred.
- BSN preferred
- Familiarity with an electronic practice management system is preferred.
