***Please note:  This position will be split between the Woodland, CA and Sacramento, CA clinic.

Position Summary:

The Ambulatory Clinical Pharmacist (CP) works under limited supervision and is responsible for a variety of clinical operations within Dignity Health Medical Foundation.
The Clinical Pharmacist provides direct patient care in non-acute clinics and coordinates services with other healthcare staff as appropriate. The Clinical Pharmacist works to maintain Pharmacy department goals while pursuing ongoing process improvement.

The Clinical Pharmacist is responsible for working within their scope of work as defined by their licensure, including:
(1) Perform patient assessments.
(2) Order and interpret drug therapy-related tests.
(3) Refer patients to other health care providers.
(4) Participate in the evaluation and management of diseases and health conditions in collaboration with other health care providers.
(5) Initiate, adjust, or discontinue drug therapy.


Minimum Qualifications:

- 2 years progressive experience in clinical pharmacy setting or post residency pharmacist.
- BS or PharmD from an accredited school of pharmacy
- Current CA Pharmacist Licensure in good standing
- Current knowledge of evidence based clinical pharmacy practice and pharmacotherapies
- Knowledge of Health Plans such as Medicare, Medical, Blue Shields, Blue Cross, HNET, AETNA, WHA, etc.
- Demonstrates knowledge and skills necessary to provide care appropriate to all age groups of patients served.
- Ability to become fully credentialed with the Dignity Health Medical Foundation and be able to obtain a California Advanced Practice (APh) license within the established time frame.

Preferred Qualifications:

- Prior experience in direct patient care under Managed Care or Ambulatory Care settings.
- Board certified specialty
- BLS certification through American Heart Association
- Clinical knowledge of specialty pharmacy; innovative pharmacy practice; pharmacoeconomics; regulations applicable to Pharmacy, including DEA, Board of Pharmacy, The Joint Commission, etc
- Pharmacy Residency Board certified specialty
- Current Vaccination certification
