Responsibilities

Virginia Mason Medical Center has an opening for a Palliative Care Social Worker to join our growing outpatient palliative care team.

This is an unique opportunity to join an established Palliative Care program whose mission is to align the care of our patients through advocacy, collaboration, education and meaningful connection.

We currently have 8 interprofessional team members who cover both the hospital and clinic settings. Our current team culture highly values compassion, communication, support and work life balance.

We are looking for a Palliative Care Social Worker who matches our current passion and enthusiasm for transforming how Palliative Care is delivered, while continuing to provide excellent specialty Palliative Care for our patients.


This position will primarily work in the outpatient setting offering specialty palliative care services to patients and families living with serious illness. Our clinic is currently embedded in our cancer center with a primary focus on supporting oncology patients. However we also accept referrals from across the system, which can include patients with COPD, CHF, cirrhosis and neurologic disorders.

Our Palliative Care Social Worker works as an independent practitioner offering palliative care consultative services as well as more specialized support when required. This could include combined visits with other palliative care team members and/or Oncology staff as well as individual sessions around goals of care, advanced care planning, psychosocial issues, counselling, and grief support. We offer both in person visits as well as TeleHealth.


Other opportunities include hospital coverage of clinic patients, education with medical residents and social work students, as well as opportunities to participate in current communication trainings including Vital Talk and the Serious Illness Conversation Guide.

Qualifications
We require:
Master's in Social Work from a graduate program accredited by the Council on Social Worker Education of Doctorate in Social Work required. Must also have an active Licensed Independent Clinical Social Worker (LICSW) license in the state of Washington and have completed four hours training in HIV/AIDS. Familiarity with EPIC/Cerner or other electronic charting system required along with exceptional communication and listening skills. 
Prefer at least 3 years experience in a clinical setting, certification as a Chemical Dependency Professional, and/or previous experience in an acute medical or mental healthcare in an inpatient or outpatient setting. 
Candidates with previous palliative care experience preferred.
