Responsibilities

Postion Summary:

The hospice/palliative care nurse practitioner works in collaboration with Dignity Hospice staff to provide comprehensive, quality hospice and palliative care services to patients under the direction of the supervising Hospice and Palliative Care Medical Director. (S)he will be responsible for providing patient visits for palliative care patients both new and established and coordinating their plan of care with other designated staff. (S)he will function as an integral member of the Hospice provider staff. The hospice/palliative care nurse practitioner will fulfill the requirements of the face-to-face encounter with a Medicare beneficiary enrolled in hospice. Patient visits will include visits to patient's homes, facilities, and hospitals. (S)he will engage in independent decision-making about patient's healthcare concerns and provide hospice or palliative care services as needed. Hospice medical director will be available for consultation, collaboration, or referral.


Qualifications

Minimum:

  • 2-4 years of experience with primary care specialty(FM, geriatrics) or sub specialty such as cardiology, neurology, or oncology with some experience with serious illness and symptom management.
  • 2-4 years of experience in a medical specialty involved in the care and treatment of patients diagnosed with serious, advanced or terminal illnesses.
  • The hospice/palliative care nurse practitioner will provide medical support and coordination to palliative care patients and family members.
  • Ability to obtain, if not certified,
  • Certification for Hospice and Palliative Advance Practice Nurses within a time period agreed by Dignity Hospice.
  • Ability to obtain, if not certified, Certification for Hospice and Palliative Advance Practice Nurses within a time period agreed by Dignity Hospice.
  • Knowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment in hospice and palliative care preferred. Knowledge of Medicare
  • Face to Face Encounter preferred
  • Ability to work collaboratively with referring, primary, consulting physicians and other members of the interdisciplinary team to provide appropriate hospice and palliative care services to patients and families.
  • Ability to work with other health care providers in long term care settings.
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  • Knowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment in hospice and palliative care preferred. Knowledge of Medicare
  • Face to Face Encounter preferred
  • Ability to work collaboratively with referring, primary, consulting physicians and other members of the interdisciplinary team to provide appropriate hospice and palliative care services to patients and families.
  • Ability to work with other health care providers in long term care settings.

  • Licensed by the State of California, Board of Registered Nurse National Certification as a Nurse Practitioner DEA
