Overview

Dignity Health Home Health Services, now part of CommonSpirit Health formed between Catholic Health Initiatives (CHI) and Dignity Health, is a dedicated team of home care and hospice professionals serving the people of San Luis Obispo County to Northern Santa Barbara County. We are a Medicare-certifiedstate-licensed agency that earned deemed-status accreditation by the Joint Commission.   

Our Hospice care involves a team-oriented approach to expert medical care, symptom and pain management, and emotional and spiritual support expressly tailored to the patient’s needs.  Our primary goal is to fulfill our patient’s wishes and to support their families and caregivers. We are seeking a Hospice Clinical Supervisor to join our team, you will report directly to the Director of Operations and help us in this mission.  You will lead a team of 10-15 include nursing, social work, chaplain, aides, volunteers, and providers.  Come join our team, grow your leadership skills, and be part of an integrated continuum of care for patients and families. 

Sign-On Bonus Eligible: $10,000

Compensation (between $101,920  - $164,320) depending on experience

Responsibilities

Position Summary:

The Clinical Supervisor provides overall supervision of direct and indirect patient care for registered professional nursing and home health aid staff. Clinical supervisor demonstrates leadership capabilities to effectively assure quality care to the patient through us of the nursing process and ensuring staff competency. Major responsibilities are listed; however, at times the Clinical Supervisor may be assigned other duties.

Qualifications

Minimum Requirements:

  • Two years supervisory experience within the last 5 years in a home health agency, primary care clinic, or health facility
  • Sufficient background knowledge and expertise in clinical decision-making for the patient population assigned
  • Current California RN license
  • Current CPR-AHA Certification
  • Current California Driver's License with proof of liability insurance for automobile.
