Responsibilities
Shift: Monday - Friday 8am - 5pm Including Weekend Rotation
Job Summary:
Incorporates a broad spectrum of knowledge in the application of nursing to provide client care in the home or place of residence. Home Health nursing is holistic and client-focused; integrating family and caregivers, environmental and community resources to promote an optimal level of client wellbeing. The LPN provides care in the home environment based on the physician ordered plan of care.
Essential Duties:
- Provides professional communication in the office to physicians, staff and patients. Maintains the highest level of client rapport and satisfaction.
- Provides and documents skilled care, treatments and procedures according to the physician’s plan of care.
- Obtains, reviews and documents written and verbal physician orders, and follows up on orders according to policy.
- Assesses and documents homebound status and skilled nursing needs each visit.
- Maintains complete, organized, current clinical records on all assigned patient.
- Reports all issues, patient concerns and potential scenarios to Supervisor immediately for resolution and further direction; follows through on issues that may compromise patient care by using appropriate chain of command.
- Assists with chart audits under the direction of the Supervisor.
- Assists with OBQI/OBQM under the direction of the Supervisor.
- Participates in agency and community programs, as requested to promote professional growth and understanding of home health.
- Achieves proficiency in department electronic documentation program, Allscripts, within 2 months of hire.
- Adapts to change in a positive and professional manner.
- Supports team members and cooperates well with coworkers, physicians and referral sources. Maintains and builds positive working relationship and fosters a cooperative work environment. Displays honesty and mutual respect, when communicating with peers, physicians and patients.
- Maintains current knowledge of Home Health regulations.
- Maintains competence required in assigned practice area and attends required educational programs as mandated to maintain licensure.
- Accurately completes all documentation in a timely manner in accordance with Mercy policies and State/Federal/Joint Commission regulations.
- Assess and identify patient needs for supplies, equipment and teaching materials. Charge appropriately for needed supplies.
- Manages assignments with productivity targeted at 5-6 patients per day.
- Attends 80% of staff meetings and all mandatory in-services.
- Completes annual education through on-line training and/or skills day participation.
Qualifications
License/Certifications:
- Current license as an LPN issued by the State of Oregon required.
- Must have current American Heart Association BLS for Health Care Providers within 30 days of hire.
Possess a valid Oregon Driver’s License, automobile insurance and provide one’s own transportation.
Experience:
- Two (2) years acute care or clinical experience required.
- Home Health and/or Hospice experience preferred.
- Must have communication and interpersonal skills in relating to public and peers.
- Ability to work under stressful situations.
Education:
- High School Diploma or GED required.
