      Hospice Care LPN - PRNJob Responsilibities:

  • Provides Hospice care and education to patients and their caregivers
  • Participates in the coordination of services with other disciplines and community resources as needed
  • Evaluates and revises care plans/assessments based on changes in patients and/or their environments
  • Participates in regular case conferences and in-services or special case orientations as needed and participates in on-call rotation

Qualifications
  • An active LPN license in the state of Nebraska
  • Graduate of an accredited school of Nursing
  • One year of LPN in a Hospice Health care setting 
  • Proof of eligibility to pass medications required
  • The ability to think and act quickly and discern the difference between urgent vs. emergency situations
  • Passion for excellence in quality of care
  • Must have good communication skills
  • Must have good interpersonal skills
