CHI Health at Home is looking for experienced Home Health Aide/CNA, that are you looking for rewarding Nursing Career path where opportunities to grow within the company is available?

If so, CHI Health at Home is looking for a CNAs to work in Home Health care, where YOU MATTER. Where you will work as a Home Health Aide/CNA asisting in daily personal and grooming services, household care and provision of nutrition. This position will work under the supervision of a Registered Nurse, Physical Therapist, Occupational Therapist or Speech Therapist.

Job Responsibilites listed, but not all included:

  • Assisting with ambulation, and assists with active or performs passive range motion exercises.
  • Communicate changes regarding patient’s physical/mental condition, completes and submits documentation in a timely and appropriate manner.
  • Participates in case conferences, attends in-services, and/or special case orientation.

  • High school graduate or equivalent.
  • One year Home Health Aide/CNA experience, patient tech or other related experience and/or passes CHI Health at Home’s competency evaluation program (Medicare regulation)
  • Must obtain and maintain CPR or BLS certified.
  • Successful completion of all competency testing.
