Responsibilities

The primary role of the Rehab Navigator is to assist with the coordination of care for the Rehab patient and their family. The Navigator partner's with the patient, family and the interdisciplinary team to provide support, education and assist with the planning and coordination of the patient's discharge to the next continuum of care. The Navigator is expected to work closely and under the general supervision of the Rehab MSW & the RN Rehab Case Manager. The Navigator sees patients ranging in age from adolescent to geriatric. The Navigator will advocate for the patient and family valuing the patient's preferences, spiritual, cultural and language needs. They will also assist with identifying any barriers affecting the patient's transition to the next continuum of care. The Rehab Navigator is also accountable to the Manager of Rehab Therapies and is responsible for adhering to regulatory standards including CARF.


Qualifications
  • Two years' experience or field placement in hospital, or healthcare related setting required.
  • High School Diploma/GED
  • Must maintain current AHA BLS card.
  • Fire Safety card required within 90 days of hire.
  • CCS paneling within six months of hire preferred.
