Responsibilities

JOB SUMMARY / PURPOSE


The RN Case Manager monitors and evaluates patient care practices in the acute and swing bed units, performs quality,
utilization, and case management functions, coordinating a patient’s care across the continuum of care. Facilitates
positive clinical and cost-effective outcomes.

ESSENTIAL KEY JOB RESPONSIBILITIES


1. Performs chart reviews for appropriate level of care.
2. Discuss discharge plans with discharge planning team and Primary Care Physician.
3. Monitor and notify physicians of observation hours and length of stay.
4. Collects data regarding core measures and provides feedback to Physicians.
5. Facilitates appropriate clinical documentation to ensure that the level of services and acuity of care are
accurately reflected in the medical record. Improves overall quality and completeness of clinical documentation.
Conducts follow-up reviews of clinical documentation to ensure issues discussed and clarified with the physician
have been recorded in the patient’s chart.
6. Assists in the medical screening process by documenting appropriateness of patient admission and computer
system as appropriate.
7. As appropriate, documents, analyzes data and reports instances of inappropriate patient care, discharge delays,
etc. to the Director of Quality Resources.

Qualifications

Required Education and Experience - Associate’s Degree in Nursing


Required Licensure and Certifications - Registered Nurse: KY

Required Minimum Knowledge, Skills, Abilities
and Training

3-5 years of clinical experience in the acute care setting
Must be able to communicate in written and spoken English.
Must be proficient with computer systems and software used in the
healthcare setting.
