Lead with Heart - Nurturing Care Coordination

In this inspiring role, you hold the power to perform patient comprehensive assessments, collaborating seamlessly with our multi-disciplinary team of experts, including PT, OT, ST, MSW, and paraprofessionals. Together, we will work passionately to achieve the heartfelt outcomes and goals of our patients, leading them on a journey of healing and hope.

Supervise your caseload, nurturing the growth and development of LPNs and Home Health Aides, as we cultivate a harmonious team of caregivers. In your role as a facilitator of care coordination, we celebrate the unity that arises when PRN staff steps in, ensuring seamless support for your patients.

Qualifications

Active RN License in the State of service

One (1+) Year of Clinical experience required; Hospital experience preferred, Home Health experience a plus

Current CPR Certification Required

Valid Driver's License
