AmeriMed provides home infusion therapy services to help patients safely transition care to their home, alleviating long and unnecessary stays in the hospital. Through our experienced and professional pharmacy team at AmeriMed we provide total care for our patients. Working directly with physicians, we are able to manage a variety of diseases and conditions in a homecare setting. Our goal is to help each patient be an active participant in their care. Through education and partnership with each patient and their caregivers, we teach our patients how to effectively manage their therapy and provide ongoing oversight, coordination and support throughout their therapy.

Join our team as a PRN Care Transition Nurse and help make a difference in the communities we serve and do what you love every day!

As a PRN Care Transition Nurse you will contribute by working as a field-based business development representative, consistently meeting the volume expectations for referrals and admissions. This position collaborates with physicians, hospitals, nursing homes and home care personnel in the process of providing coordinated quality home based services for patients referred to Community Health at Home. This activity may be in a health care facility or in the field office location.

Day-to-day, your role includes:


• Follows referral of a patient to Community Health at Home, assists discharge coordinators in coordinating quality home care services for clients in the following manner
• Determines patient eligibility for home care services
• Develops professional working relationships with health care providers and facilities, generating an open flow of information and support of home care goals; provides value added counsel, teaching and resourceful  problem solving.
• Available to all discharge planners, physicians, other hospital personnel and clients to 1) analyze eligibility for home care or hospice and provide general guidance in determining if a patient would benefit from home care or hospice services and/or 2) explain Medicare and Medicaid guidelines, insurance plan benefits, financing options, Community Health at Home policies, etc.
• Assists in ordering equipment, oxygen, IV’s, etc., as required to provide proper home care for the patient referred to Community Health at Home
• When requested, participates in the education of providers and personnel on home care regulations and the variety of services available as well as industry trends.
• Participates in Quality Assurance Performance Improvement (QAPI)


Qualifications

  • Graduate of an accredited school of nursing. Current RN licensed to practice in the state of Indiana.
  • Two years of appropriate nursing experience is required.
  • Experience as a Care Transition Nurse or similar role, sales or marketing, or customer service preferred.
  • Hospital and/or homecare nursing experience is preferred.
  • Currently licensed to practice in the state serviced.
  • Excellent clinical skills.
  • Strong written and verbal skills.
  • Ability to work closely with a wide range of professionals or a multi-disciplinary team.
  • Interpersonal skills that facilitate inter- and intra-team/departmental communication.
  • Excellent organizational and time management skills.
  • Excellent attention to detail.
  • General business computer skills proficiency and familiar with multiple software programs including Windows-based programs.
  • Valid Driver’s/Chauffeurs license and auto liability insurance coverage according to organizational policy.
