Overview

Sierra Nevada Memorial Hospital has proudly served as the sole healthcare provider for western Nevada County for more than 50 years. Much has happened since the hospital opened in 1958, including additions in 1994 of a 68,000 square foot Outpatient Center, a comprehensive Cancer Center and most recently, the opening of the Sierra Nevada Diagnostic Center in the fall of 2006. SNMH is a licensed, acute care hospital providing a full range of services including cardiovascular, surgical, emergency and diagnostic. SNMH offers access to the SNMH Health Sciences Library. The library features continuing and medical education for healthcare professionals, diabetes classes, prenatal classes, cancer support groups, and family caregiver classes. In addition, we have more than 90 board certified primary and specialty care physicians on our active and associate medical staff.

POSITION SUMMARY:

The Home Care Physical Therapist is responsible for providing physical therapy services to Home Care patients.

JOB SPECIFIC RESPONSIBILITIES AND DUTIES
1.1 Provides physical therapy services to patients/clients according to a written physician's plan of care. This may include,
but is not limited to: Assessing, evaluating, and updating therapeutic/rehabilitative/functional status, and participation in
the development of the total plan of care. Evaluates home environment. Assesses for muscle strength, mobility, gait,
ROM potential for rehab. Directing physical therapy treatment. Instructing patients/clients and families/caregivers in the
use and care of therapeutic appliances. Determining priority needs for physical therapy. Reporting the patient's/client's
reaction to treatment or changes in condition to the physician.
1.2 Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of
physical therapy with which they may work with a patient/client, as well as instructing them as to the goals of the physical
therapy program for the patient/client by participating in case conferences.
1.3 May train patient/client in the use of prosthetic device.
1.4 Supervising homemaker/Home Health Aides when such personnel are participating in patient's/client's rehabilitation
regime.
1.5 Identifies patient/client and family/caregiver needs for other home health services and refers as necessary.
1.6 Prepares and submits clinical and progress summaries based on the attainment of goals, observing, recording and
reporting information on the patient's condition to the attending physician and in the patient's health record.
1.7 Prepares and submits a clinical progress summary based on the attainment of goals directed by agency policy.
1.8 Provides physical therapy consultation to home families/caregivers when indicated.
1.9 Provides inservice education programs for nursing agency personnel as needed.
1.10 Participates in peer consultation process.
1.11 Participates in and completes ongoing staff competency assessment.
1.12 Complies with and understands Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA and other regulations and
licensure requirements as they pertain to their position.
1.13 Performs other duties as assigned and/or requested.

Qualifications

REQUIRES

  • Possesses and maintains good physical stamina and mental health.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or agency requirements and is in good working order.
  • Good oral and written communication skills; has good organizational skills.
  • Current PT Lic, Current CPR, Current Driver's Lic, Current Car Ins.

PREFERS:

  • Two years of appropriate experiences as a Physical Therapist.
  • Community/home health experience is preferred.
