The Certified Wound Ostomy Nurse (CWON) is responsible for collaboration in developing and coordinating the home health agency wound and ostomy care program. This CWON is responsible for working home health and hospice and assisting with complex wound/ostomy patients. This CWON participates in establishing standards of care and clinical competencies to meet patient needs and training to ensure clinical expertise. The incumbent collaborates with physicians, clinical staff, administration and profides education to patients, caregivers, and staff on wound, ostomy care. This position requires the full understanding and active participation in fulfilling the mission of Dignity Health/ CommonSpirit Health at Home.

Qualifications

$5,000 - Sign On Bonus offered for new hires. Per policy, current employees are not eligible.

Minimum:

  • Current CA RN licensure & BLS from the American Heart Association (AHA)
  • 3 years nursing experience within the last 5 years 
  • Certified Wound Ostomy Nurse (CWON) or Certified Wound Ostomy Continence Nurse (CWOCN) certification
  • Current CA driver's license. If out of state license, consistent with CA DMV regulations, must obtain CA driver's license within the first 30 days of employment
  • Current possession of automobile insurance and access to an operating motor vehicle
  • Verbal and written communication and presentation skills, and computer skills required
