Responsibilities

This Social Work position provides coordination and delivery of psychosocial services and works collaboratively with the physician, multidisciplinary team,and the patients throughout the continuum, including prevention, short-term counseling, and behavioral health diagnosis support through the lifestyle journey. Provides psychosocial services designed to foster coping, support optimal functioningin conjunction with the skills learned in the programand maximize the patient’s utilization of available resources. Participates in program development, implementation and promotional activities in a variety of settings.

Essential Key Job Responsibilities

  • Demonstrates competency in clinical knowledge and skills in assessing psychosocial consequences of the population in a geriatric setting, as well as implementing appropriate treatment plans.
  • Develops a comprehensive bio-psychosocial assessment and intervention plan responsive to the patients and their families, staff needs and concerns.
  • Develops an effective working relationship with patient/family to engage, collaborate, advocate and problem solve in order to enhance their functioning.
  • Utilizes, as a treatment modality, individual, couples and family and/or group psychotherapy.
  • Demonstrates competency in accessing community and pharmaceutical resources to provide financial, transportation and supportive assistance.
  • Directs patients and their families to financial/psychosocial resources available to them that are specific to geriatric diseases and diagnosis, including resources for their children and family members.
  • Provides end of life conversations, assists patient/family with decision-making and transitioning.
  • Provides bereavement support and refer to outpatient grief support groups.
  • Coordinates with appropriate staff to provide for transition from inpatient to outpatient status or from outpatient to inpatient status.
  • Maintains an up to date, accurate and appropriate record of social work activities in patient’s medical chart which does not violate confidentiality.
  • Provides ongoing counseling and support to all physician-identified patients and their families.
  • Commits to continuing education regarding geriatric diseases and treatments, psychosocial implications for patients and families, appropriate interventions, and available community and governmental resources.
  • Maintains research-based knowledge of psychosocial issues related to geriatric diseases, interventions and outcome, including integrative therapies.
  • Collaboratively develops and implements patient/family education programs and literature.
  • Provides education of staff related to an individual and/or family’s psychosocial needs.
  • Participates with the Practice Team in developing and implementing programs and services in a variety of settings.
  • Demonstrates initiative and leadership in developing services which respond to patient care demands.
  • Follows policy and procedures, NASW Code of Ethics, ACSW Standards of Practice and any other governmental and/or regulatory policies, rules and regulations.
  • Proficient computer skills, including but not limited to Microsoft Office suite applications such as Word and Excel.
  • Demonstrate business and analytic skills.
  • Demonstrate effective verbal and written communication and organizational skills
  • Demonstrate ability to shape communications to the needs of the audience.
  • Perform other duties as assigned.

Qualifications

Required Education

  • Masters of Science in Social Work (MSW) is required

Required Licensure and Certifications

  • Unrestricted License by the State of Tennessee (LCSW) is required.
  • Basic Life Support (BLS) or Advanced Care Life Support (ACLS) required within 6 months of hire date.
