Responsibilities
CHI Health at Home is looking for a Licensed Medical Social Worker for the Lincoln, Nebraska area!

- Reports changes in patient condition to appropriate personnel and the attending physician.
- Assists the physician and other caregivers in understanding the significant social, emotional and economic factors influencing the patient’s ability to improve medically.
- Provides patient care education to patient and other caregivers.
- Works with the patient and caregivers to resolve problems interfering with an optimum caring relationship between the patient and caregiver.
- Assists in making appropriate community referrals to gain access to additional resources as needed. (i.e. applications for public assistance, housing problems, additional health care resources, transportation).
- Participates in agency in-services, case conferences, discharge planning, summary reports, quarterly
record review and annual program review.
A compensation range will be based off a part time work schedule, which is the reasonable estimate that CHI Health at Home in good faith believes it might pay for this particular job based on the circumstances at the time of posting. CHI Health at Home may ultimately pay more or less than the posted range as permitted by law.
Qualifications
- Masters degree from an accredited school approved by the Council on Social Work Education.
- 6 months to 1 year Home Health and/or Hospice.
- Currently licensed/certified (if required) to practice in the state serviced.
- One year social work experience in a health care setting required. Home care experience preferred.
- Must obtain and maintain current CPR Certification.
- Valid Driver’s License
- Intuitive knowledge of technology and comfortable with smartphones, tablets, virtual reality, interactive software and artificial intelligence
