Roseburg is located on Interstate 5 in Southern Oregons scenic Douglas County. The community is nestled in a lush forested region known as the 100 Valleys of the Umpqua. Responsibilities

Shift: 8am - 5pm

Core Schedule: Monday - Friday, Includes Call Rotation

Job Summary:

The Hospice RN incorporates a broad spectrum of knowledge in the application of nursing to provide client care in the home, place of residence or appropriate community site. It is not limited to a particular age group or diagnosis. Hospice nursing is holistic and is focused on the individual client, integrating family/caregiver, environmental and community resources to promote an optimal level of client and family well-being. The framework of practice is care management with an emphasis on end of life care. The Hospice RN possesses a broad knowledge base in Hospice care with expertise in pain and symptom management. Care management includes: use of nursing process to assess, diagnosis, plan and evaluate care. Performing nursing interventions, including teaching, coordinating and using referral and resources, providing and monitoring all levels of technical care, collaborating with other disciplines and providers, identifying clinical problems and using research knowledge, supervising ancillary personnel and advocating for the client’s right to self-determination.

Essential Duties:

  • Assess and develops a plan of care through interviews of patient and primary caregivers/family, performs a physical assessment, integrates physical assessment with previous health and psychosocial history.
  • Provides case management and documents skilled care, treatments and diagnostic procedures according to the physician’s plan of treatment and nursing care standards.
  • Demonstrates responsibility for primary nursing management of assigned patients (i.e. communicates with intra/interagency personnel to provide continuity of care).
  • Completes and documents initial evaluation of assigned patients. Assesses patient status and skilled nursing needs on admission and each subsequent visit.
  • Prepares recertification, discharge summaries and all federally required data entry and/or forms for Medicare.
  • Regularly evaluates the patient’s needs and informs physician and the supervisor of change of the patient’s condition, and provides necessary revision in the plan of treatment. Makes referrals as appropriate and authorized by physician. Coordinates services through consultation and interdisciplinary conferences.
  • Assess and identifies need for supplies, equipment and teaching materials for current and next visit in computer. Documents and charges appropriate for supplies.
  • Completes daily time log with supporting documentation in electronic record for each day worked, and synchronizes laptop to the home system each morning, after any schedule changes and at the end of each work day.
  • Utilizes knowledge of Medicare, Medicaid, TJC, OSHA, & NHPCO regulations pertaining to Home Health/Hospice.
  • Participates in call evenings (including call and after 10pm visits for home health) and weekend call and rotation.
  • Demonstrates understanding of Hospice program and philosophy and standards of care.
  • Sets up weekly schedule for all patients and enters into electronic schedule by 5:00 pm Thursday.
  • Manages assignments with productivity targeted at 5 pt. visits per work day for Hospice, special considerations may be given for geographic areas and patient acuity.
  • Demonstrates competence in use and care of laptop computer.
  • Participates on at least one Home Health/Hospice committee.
  • Attends 80% of the staff meetings and all mandatory in-services.
  • Completes annual education day by computer or in-service. Earns 10 CEU hours per year.

Qualifications

License/Certification:

  • Current RN license issued by the State of Oregon.
  • Must have American Heart Association BLS for Health Care Providers within 30 days of hire.
  • Must possess a valid Oregon Driver’s License, automobile insurance and provide own transportation.

Experience:

  • Two (2) years acute care nursing experience required.
  • One (1) year  hospice nursing experience or two (2) years home health nursing experience preferred.
  • Must have flexible schedule and ability to meet on-call requirements of the position.
  • Must have good assessment, communication, and documentation skills.

Education:

  • Associate’s Degree in Nursing required.
